Our Process

The First Call

Getting services started with Old Dominion Home Care typically begins with a phone call to our office or an e-mail requesting information. Often one of the owners or a Registered Nurse will begin the process of listening to the client or the family member to get an understanding of the type of care that is needed, as well as the expectations.  We will of course answer any questions you may have at this time, as well as encourage you to set up an in home assessment.


Individual Plan /In-Home Free consultation

Once the initial intake over the phone occurs, we will set up a FREE assessment to meet with you at your home, hospital or another facility.  Ideally, it is important to meet the client in their home for many reasons, but primarily this is where they will be most comfortable talking to us, and having us assess their physical and mental needs.  In addition, A Home Safety Assessment will be conducted.

During the initial assessment, a Plan of Care will be customized for the client’s specific needs, and suggestions as to what may be an appropriate schedule will be discussed.


Caregiver Selection

After the initial assessment takes place, the scheduling team meets with the RN or the owner to review the specific client needs and expectations. This is the point where we begin the process of “matching” the best caregiver for the client.

In order to insure the best match of caregivers to our clients we take into consideration: the plan of care required, the personalities of the client and caregiver, the geography of client, and the schedule requested.

If for any reason, you are not satisfied with the caregiver, please contact the office, and we will happily make a change.  It is important for us to provide the best care possible with the best caregivers for each client.


Safe Transportation

Transportation is standard for our home care services.  All of our caregivers have valid drivers licenses, carry auto insurance, and have had their driving record reviewed.  We are able to take our clients to doctor’s appointments, grocery shopping, visit family/friends, run errands, attend religious services, etc.

In addition, we also can provide Transportation to clients who may not need our typical home care services, but may need transportation only.  We can provide transportation for people that need to be driven to chemo therapy, radiation or dialysis treatments.  Transportation will be provided in the caregivers or clients car, and the client must be able to get into the car with minimal assistance.  We are able to put a folding wheelchair into the car if needed; however, we do not provide handicapped vehicles.


Quality Assurance

Once services begin, we periodically conduct quality assurance phone calls and unannounced visits to our client’s homes, as well as supervisory visits/follow-up assessments by our nursing team. The owners are involved in each step of this process to help insure that we exceed the client’s expectations.  Communication with our families, as well as with our caregivers in the home, is very important.

Services are available as little as (12) hours per week, or up to 24/7 care every day of the year.
